HCU Senior Care Overview

At Health Care Unlimited (HCU), every patient is important, regardless of the complexity of care, age, or payer source. Our commitment to the patients and to the integrity of our business drives the daily mission of team members.

While our primary goal is to help patients and their families regain some or most of their independence from a medical condition, there are programs HCU offers that are designed to assist patients, health care professionals and family members of patients which have a progressive medical condition. Ultimately, these programs are designed to help mitigate the loss of an independent lifestyle and bring comfort in the knowledge that there is another helping hand concerned about the patient’s wellbeing.

Senior (Geriatric) Home Health Care

Home health care includes a broad range of periodic or intermittent skilled nursing care including education and disease management, and other skilled care services such as physical therapy, occupational therapy, and speech therapy. Services may also include medical social services or assistance from a home health aide.

Infusion Pharmacy

As part of any Senior Care Program, our Infusion Pharmacy is standing by to provide patients with access to medications that are delivered using intravenous therapy through an access device such as peripheral needles or central IV lines.

In-Home Rehabilitation

Therapy for patients with physical (PT), occupational (OT), and speech and language (ST) needs is available in the home. PT improves mobility, range of motion, muscle strength, reduces chronic pain and restores balance and coordination. OT increases independence with everyday living and household tasks such as eating, grooming and dressing. ST assists patients with difficulty swallowing or other memory problems as well as learning/regaining the ability to communicate.

Senior (Geriatric) Private Duty Services

Geriatric Private Duty Care is available for those who require skilled (professionally licensed) or custodial (non-medical) care. Non-medical care can be provided by a nurse, but is most often provided by assistive personnel such as nursing assistants, companions, sitters or attendants. Private Duty services provide care from 4 to 24 hours per day and can be paid by Medicaid, long-term health and insurance plans as well as individual private pay.

Senior (Geriatric) Attendant Services

Attendant Services are personal care and assistance services provided to a client with functional limitations due to a medical condition. Services can be custodial in nature as well as skilled depending on the client’s qualifications and conditions. Typical services include assistance with activities of daily living.

Nilda Sanchez
Director of Skilled Nursing Services

As the Director of Skilled Nursing Services, Nilda Sanchez oversees the departments where clinicians are employed, such as Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) and supervises the care of all patients at Health Care Unlimited. Ms. Sanchez’s home care management experience spans Private Duty Nursing and Skilled Intermittent Care for seniors, adults, children and infants. Ms. Sanchez attended University of Texas-Brownsville where she earned a Bachelor’s Degree of Science in Nursing. Ms. Sanchez brings broad post-acute healthcare knowledge and over 17 years of experience working as an RN.

What is Senior Home Health Care?

Senior Home Health Care also known as Geriatric Skilled Intermittent Service is characterized as a service in which a skilled health care professional such as a Registered Nurse (RN) and others listed below, visit the patient’s home to provide periodic (intermittent) care for a short timeframe. These “skilled” visits serve as part of a health care intervention regimen designed to help stabilize and monitor the patient with the expectation of returning them to an independent lifestyle where he/she or their caregiver(s) can continue to manage their care under the supervision of their physician. Geriatric is a medical classification typically given to an elderly person usually above the age of 65.

Our geriatric home care team work to help elderly patients reach their maximum potential for clinical and skilled care independence. They focus on long-term and skilled chronic or post-acute care, medical illness, diseases, and conditions where skilled nursing procedures may need to be performed in the home and/or taught to a family or caregiver.

What are Geriatric Private Duty Services?

Geriatric private duty services in the home care setting are characterized by long-term care needs of seniors or families. These services can be paid privately or covered under insurance plans. Skill level can range from sitter services provided by a personal attendant to 24-hour skilled nursing care provided to a geriatric patient by registered nurses (RNs).

Geriatric nurses need effective assessment skills. They must know the differences in development from adult to elderly. Our nurses are compassionate and empathetic, sensitive to patient’s needs, and be able to teach parents. Nurses also provide follow-up information to families for insulin, oxygen use, nebulizer use, and other treatments. HCU pharmacists can provide any special IVs and drugs a patient needs.

Elderly patients with progressive medical conditions, which are manageable but not curable, often require daily assistance due to cognitive and functional limitations and/or degeneration. Their quality of life is so much better thanks to the continuum of care HCU geriatric nurses provide as they coordinate with doctors, hospitalists and other primary caregivers.

For senior patients requiring constant supervision or assistance with activities of daily living such as dressing and light meal preparation, Health Care Unlimited offers Senior Attendant Services. Senior Attendant Services can range from companionship to assistance with activities of daily living such as bathing, grooming and eating. Additionally, this care can be provided by a Personal Care Attendant or a Certified Nurse Assistant (CNA).
