Skilled Nursing
Health Care Unlimited’s caring, skilled staff is made up of Registered Nurses and Licensed Vocational Nurses, available 24 hours a day, 7 days a week.
We provide general and specialized nursing care to individuals at home, under the direct supervision of the patient’s physician. We work closely with your doctor to give you the professional care and services that you have come to expect from our team of professional health care providers. The wide variety of specialized nursing care allows us the flexibility to care for all of your health care needs. We offer highly trained nurses to assist with all aspects of nursing care for example, wound care, disease process management, and we do specialize in intravenous therapy. Because we do have our own infusion pharmacy, this makes coordination of your care that much easier.
Most of our nursing staff will utilize a point of care device, or “Life Book” to document your care. This mini computer is taken into your home to assist the nurse in documenting your health status, progress and needs. Your physician may view your health care information and progress at any given time. Any other disciplines involved in your care will be aware of your status and progress, this will assist in better communication and build positive patient outcomes.
Physical and Occupational Therapy
There is nothing more frightening or frustrating than being unable to perform such easy tasks as combing your hair, dressing yourself, or walking to the bathroom. These activities can be affected by many different types of illnesses, arthritis, a stroke, a car accident to name a few. In any case, a Physical Therapist may be ordered to care for your needs. A qualified therapist will perform a through evaluation and develop a plan of care under the direct supervision of your physician.
The plan of care may include such activities as gait training, home exercise program, which will assist with increasing strength, endurance and flexibility. This will assist with your recovery
An Occupational Therapist may also be ordered by your physician to assist with fine motor skills. This may include things like tying your shoes, putting on your socks, combing your hair or even brushing your teeth.
Speech Therapy
A Speech Therapist may also be ordered by your physician to assist with speech articulation and/or problems with swallowing. The therapist will assist with treatments and exercises designed to improve dysphasia (difficulty swallowing) and speech. Other treatments may include language disorder treatments and speech articulation disorder treatments and the use of non- verbal communication skills.
Medical Social Work
Our social workers provide counseling on community assistance, financial resources, and can also assist with emotional support for patient and family. Our social workers can help identify patient’s social and emotional factors related to patient illness and the need for care. Transportation assistance for medical care, delivery of meals, and assistance with the purchase of medications are just a few of the resources our social workers may contact for assistance.
Home Health Aides
For those patients needing help with their activities of daily living our certified home health aides will assist patients according to their health needs. Our home health aides can assist with bathing, meal preparations, dressing, light housekeeping and other activities of daily living. A qualified clinician will develop a plan of care appropriate to each individual patient’s needs and will regularly supervise the home health aide throughout your home care episode to ensure your health care needs are being met.
Qualifying Criteria
For the delivery of “skilled services”, you must be under the care of a physician
Definition of Skilled Care Services- services that must be performed by a licensed person, i.e. nurse or therapist.
For Medicare to pay for your services, you must:
Be Medicare eligible
Be homebound
Be under the care of a licensed physician
Require services that are reasonable and necessary for treatment of an illness or injury.
For Medicaid to pay for your services, you must:
Be Medicaid eligible
Be homebound
Be under the care of a licensed physician
Require services that are reasonable and necessary for treatment of an illness or injury.
For Most Private Insurances:
Some insurance do require patients to be homebound
Be under the care of a licensed physician
Require services that are reasonable and necessary for treatment of an illness or injruy.
Private Pay:

Payor Sources
For your convenience, Health Care Unlimited, Inc. accepts payment from Medicare, Medicaid, and Private Insurance. Private payments are also accepted directly from patients/family. Insurance verification is available from our trained professionals. Our staff will ensure that you know exactly what your responsibility may be. Whether it is a co-payment, or meeting a deductible you have the right to know and we will honor your rights. All patient responsibility amounts will be explained to you verbally and in writing. Your services begin when you are satisfied with the information and responsibilities that you or your loved one may incur.
Referral Sources
Referrals to Health Care Unlimited, Inc can originate from almost anyone, however a physician must ultimately authorize the home health care.

Studies have shown that recovery occurs faster in the home. Within 48 hours of being released from the hospital, a trained Health Care Unlimited licensed professional will come to your home and conduct a thorough assessment comprised of over 100 questions. It will assist us to better take care of you or your loved ones.
The family is an important part of your recovery, and Health Care Unlimited teaches them procedures and techniques to help you.
